Upon scheduling your first appointment you will receive email with our invitation to our OnPatient Portal. Please sign up for the portal and complete the Health Profile.
Please arrive 15-20 minutes prior to your appointment to ensure your profile is set up correctly, including having your photo taken, correct insurance information has been uploaded, and all mandatory consent forms have been signed.
The first appointment will last 45-60 minutes. Your provider will review all prior health records, imaging and perform a comprehensive evaluation to determine the most effective course of care to meet your specific needs. Learn More
Upon arriving please check in with our front desk staff. We would love if you would Check In via the ONPATIENT app. This ensures we are best prepared for your arrival
Please arrive 10 minutes prior to your appointment to ensure we can update your profile with any changes prior to your appointment. Please make sure you let the front desk staff know if you have suffered a new injury or have any new concerns you would like addressed.
If insurance benefits are available for our services, we will send claims to the insurance company. We accept General Insurance, Auto Accident Claims, & Worker's Compensation Claims.
